Mexico Cofepris Medical Device Registration Service For IVD Device

Scope and evidence must be confirmed before any paid tests or filings.

A COFEPRIS registration service for an in-vitro diagnostic (IVD) device should deliver a complete, reviewable Mexican filing—not merely a generic medical-device checklist. The file must identify the exact assay, specimen, intended users, instrument or platform dependencies, claimed performance and commercial presentations.

Begin with an IVD product-definition sheet: manufacturer and manufacturing sites; analyte and intended clinical purpose; qualitative or quantitative output; specimen type; target population; professional or self-test use; kit, controls and calibrators; models/SKUs; approved markets. These facts determine what the applicant can credibly file.

Select the filing path against the actual IVD, not its overseas class

COFEPRIS publishes separate procedures for ordinary registration, recognized-authority reliance and certain low-risk devices. Determine classification and coverage against current Mexican instruments; a foreign clearance alone does not decide the Mexican route.

Route / scenarioFiling decision
Standard registration (COFEPRIS-04-060)Use the applicable standard dossier where the particular IVD does not qualify for an abbreviated or low-risk route. The public procedure requests a completed FF-COFEPRIS-21, fee evidence and scientific/technical support, subject to applicable conditions.
Reference-authority route (COFEPRIS-04-050)Consider only after matching the foreign authorization, regulatory authority, device identity and current equivalence or reliance criteria. It remains a Mexican application and decision, not automatic recognition.
Low-risk procedure (COFEPRIS-04-070)Check the actual current low-risk listing and the corresponding procedure before using this route. Do not infer eligibility just because the product is a rapid test or non-invasive.
Possible registration exemptionVerify the precise product against the current COFEPRIS low-risk / no-registration lists and its intended use. A device-family label or competitor's exemption is insufficient proof.

Decision gate: record the cited Mexican basis, risk classification, grouping rationale and route-specific document list in the filing instruction before paying fees or commissioning translations.

Allocate the applicant, signature and evidence responsibilities

A foreign manufacturer, the Mexican applicant or registration holder, and a distributor may be different organizations. Confirm the actual applicant and their authorization to submit and receive notices before preparing any signed form.

Responsible partyAction and evidence
IVD manufacturerApproves the intended use and model scope; supplies truthful design, manufacturing, analytical and clinical-performance evidence; signs the manufacturer documents applicable to the chosen route.
Mexican applicant / registration holderConfirms legal identity and representation powers, controls the application and payment evidence, files through the channel indicated for its homoclave, and tracks COFEPRIS notices and the registration decision.
Regulatory dossier preparerMaps documents to the current official checklist, maintains version control and a gap register, prepares Spanish-facing materials, and drafts responses for applicant/manufacturer approval.
Importer / distributorConfirms Mexican commercial-chain details, packaging and import arrangements, and does not treat a pending application as permission to market the IVD.

Build an IVD dossier that can be cross-checked

The published COFEPRIS registration fiche distinguishes administrative proof, technical/scientific information, Spanish labelling and additional evidence for imported products. The table below is a practical dossier index; the precise mandatory set depends on the current route and product.

Dossier segmentContents to verify
Administrative and authorityApplicant identity and powers (notarial instrument or RUPA as applicable), completed current FF-COFEPRIS-21 and proof of the applicable fee; establish the notification contact and responsible signatory.
Product identity and manufacturingTrade/generic names, intended use, assay principle, analyte, specimen, kit constituents, instrument/software compatibility, catalogue numbers and variants; manufacturing overview and relevant manufacturing-practice evidence.
Analytical evidencePerformance substantiation matched to each claim: for example analytical sensitivity, precision, specificity/interference, cross-reactivity, measuring range, stability and quality-control evidence where relevant to the IVD. These are evidence-planning headings, not an assertion that every study is mandated for every IVD.
Clinical and use-context evidenceSupport the clinical claim with available performance evaluation and study reports appropriate to the intended population and setting; reconcile claimed cut-offs, comparator methods, limitations and interpretation instructions.
Mexican-market and foreign-origin recordsDraft Spanish label and applicable Spanish instructions; for imported products review free-sale or equivalent certificates, manufacturing-practice documentation and manufacturer representation documents where the official procedure requires them. Check legalization/translation rules document by document.

Consistency control: the registered identity, instrument, analyte, specimen, intended use, package insert, performance reports and foreign certificate must describe the same commercial product. A CE mark or FDA authorization is evidence to assess, not itself a Mexican registration.

Submit, answer questions and close the filing

  1. Freeze the product-definition sheet and identify whether kit components, controls and multiple SKUs can be included together; document any grouping uncertainty.
  2. Select the current homoclave and applicant, then download its live procedure fiche, forms and guidance; verify whether its permitted filing channel is physical or electronic.
  3. Build the evidence index, obtain originals and valid manufacturing/foreign-origin records as applicable, reconcile Spanish labelling and approve translations.
  4. Complete and sign the applicable form, calculate the current official fee for the confirmed class and submit via the permitted channel; retain receipt, file number and a frozen dossier copy.
  5. Track formal COFEPRIS requests; map each question to a named document owner, supply a coordinated response in the prescribed manner, and retain correspondence and deadlines actually notified.
  6. Check the issued decision against the model list, intended use, holder and conditions before treating a registration as available for the intended commercial activity; archive the final approved records.

Common IVD-specific filing failures and how to prevent them

Failure modePreventive control
Specimen or intended-use mismatchA serum study does not silently support a saliva claim. Use a claim-to-evidence matrix and resolve discrepancies before filing.
Kit and accessory groupingMixing instrument, reagent, control and calibrator presentations without a documented grouping basis can obscure what is being registered. Map every SKU and claim to the proposed scope.
Unproven abbreviated-route eligibilityA foreign authorization for a similar version may not support the particular Mexican kit. Cross-check authority, authorization status, model, intended use and current reliance rules.
Document and language mismatchExpired or incompatible origin/manufacturing records, untranslated mandatory information and inconsistent Spanish label claims can prompt questions. Version-lock and verify each item.

Maintain the registration after COFEPRIS decides

  • Change control — Before changing a manufacturer, assay claims, specimen types, models, label or registration-holder details, assess whether an administrative or technical modification is required; COFEPRIS has separate modification procedures.
  • Post-market file — Establish a system for complaints, adverse events, field actions and device traceability, including the applicable Mexican technovigilance obligations; keep performance and quality records available.
  • Validity and renewal — Calendar the registration expiry and check the current device renewal procedure (COFEPRIS-04-090) and applicable filing rules early. This article does not promise a fixed approval period or government fee.

What a scoped IVD filing service should hand over

The engagement should specify a signed-off product/route memo, applicant-and-authority matrix, indexed IVD dossier and discrepancy register, Spanish label/IFU review, submission evidence, inquiry-response log and final registration/scope review. Manufacture, laboratory validation and regulator approval are not represented as services already performed.

For non-IVD generic submissions, see the general registration execution page. For early regulatory strategy before the product definition is final, consult the separate advisory page.

IVD registration: five practical questions

Does an FDA-cleared or CE-marked IVD automatically qualify for COFEPRIS reliance?

No. Test the actual authorization, reference authority, Mexican pathway and identical product scope against the latest official criteria; recognition never dispenses with a Mexican decision.

Can several assays and controls be placed into a single filing?

Not automatically. Prepare a SKU, reagent, calibrator, control and intended-use matrix, then assess admissible grouping under the applicable Mexican criteria before defining the filed scope.

Is a foreign manufacturer's ISO 13485 certificate sufficient on its own?

No. Manufacturing-practice evidence may be part of the dossier, but the file also needs the required application, product evidence, labelling and route-specific documents. Check the current accepted certificate conditions.

Can a product be shipped for sale while the COFEPRIS application is under review?

Do not assume that submission authorizes sale or import. Confirm the applicable registration status and any separate import requirements for the proposed activity before commercial shipment.

What should we do if the specimen type or intended use changes after approval?

Stop treating the existing scope as automatically covering the change. Compare the approved registration and revised claims, determine the relevant technical or administrative change procedure, and obtain a case-specific regulatory assessment.
